The procedure typically takes 1-2 hours, depending on the complexity. The incision is closed with dissolvable sutures after the excess breast tissue is removed.

Treatment begins with a male breast reduction consultation. Dr. Mordkovich evaluates the patient to assess the grade of gynecomastia and skin elasticity. He also reviews the patient’s medical history and any medications or supplements they are taking. Blood work and other tests may be ordered to ensure safety for surgery.

Anesthesia

Local anesthesia is used so that the patient remains awake during the surgery. A mixture of lidocaine and epinephrine is injected into the breast tissue to numb the area and minimize bleeding and bruising. The anesthetic takes effect within minutes.
Small incisions are made along the border of the areola or in the armpit area. Dr. Modkovich places them in inconspicuous places to minimize scarring.

Using a liposuction cannula (thin tube) and scalpel, the surgeon removes excess fatty and glandular breast tissue through the incisions. This is all done under the numbing effect of the local anesthesia so the patient does not feel any pain. In cases of severe gynecomastia, the surgeon may also remove excess skin.

Once the breast has been contoured to the desired shape, Dr. Mordkovich closes the incisions with sutures. These may include absorbable sutures that do not need later removal. Steri-strips or surgical tape can also be placed over the closed incisions.

The recovery period after awake gynecomastia surgery typically lasts around 6 weeks. During this time, patients are advised to avoid strenuous activity and heavy lifting to allow the incision sites to heal. They may experience residual swelling, bruising, and soreness around the chest area for a few weeks following the procedure.

They are also instructed to wear a compression garment over the treated area to minimize swelling and promote proper healing. By around 6 weeks, most of the symptoms subside, and patients are able to resume their normal activities.
